Employee
1.) An employee who needs a minor accommodation to perform the essential functions of his/her job may work with their supervisor to achieve a reasonable accommodation. For accommodation requests of a significant nature, the employee should submit a Request for Accommodation form to the Director of HR and Risk Mgt/EO-AA/IX Compliance Officer (forms attached). The employee or supervisor may defer to the Director of HR for advice or guidance at any point.
2.) It is not necessary to provide medical documentation when an employee first submits an accommodation request form. (Federal law prohibits employers from requesting genetic information of an employee or an employee’s family member unless an exception applies.) If disability related documentation is considered necessary, the employee may be required to provide documentation of the condition from a medical provider. If further documentation is needed, the HR Director will request it from the employee. Such documentation may include diagnosis, severity of condition, expected progression of symptoms, etc., to the extent such information can be provided.
3.) The HR Director will approve the accommodation that is most appropriate for both the individual and the employer. Whereas the individual’s preference will be given consideration, the University of Montevallo is free to choose among equally effective accommodations.
4.) If a request is denied, or does not result in an accommodation that the employee regards as reasonable, the employee may file a grievance with the ADA/504 Compliance Officer:

Applicant for Employment
1.) Individuals with disabilities who require a reasonable accommodation in order to respond to an announcement and/or participate in the application process must contact the Office of Human Resources at 205-665-6055. The text telephone (TTY) is available upon request.
2.) Requests for accommodation received from an applicant during the interview process should be immediately forwarded to HR for proper assessment and appropriate accommodation implementation.
3.) If phone interviews are scheduled, all applicants must be informed about the availability of reasonable accommodations. The interviewer may ask, “Do you need any special accommodations to participate in the interview?” If the response is yes, refer to number 2.
