Summary Checksheets


SUMMARY CHECKSHEET FOR INTERNS

         1.      Purchase and study the internship handbook.

         2.      Plan personal schedule to accommodate professional seminars prior to and during the internship.

         3.      Attend initial orientation session.

         4.      Check with University Meal Service regarding
                    lunches.

         5.      Meet with Cooperating Teacher to complete
                    the Intern Expectation Form.     

         6.      *Give Intern Information Form to the main office assistant/secretary, the Cooperating Teacher, and the University Supervisor.

         7.      *Return Intern Information Form and Week-By-Week Plan to University Supervisor by the end of the second week of the internship.

         8.      *Visit as often as possible with the Cooperating Teacher for the purpose of planning and observation before the internship actually begins.  Be sure to schedule visits with the teacher.

         9.      Submit all required lesson plans on time and to those who are designated to receive them.

         10.    Keep notebook (Seconday/P-12) and journal (ECE/Elementary) current and complete as described in the handbook.

         11.    **Plan and display a minimum of three bulletin boards - one with the unit, two in other areas.

         12.    **Present teaching unit to Cooperating Teacher and University Superviosr at least two weeks in advance of teaching.

         13.    **Develop a portfolio as described in the ECE/Elementary section of this handbook.

         14.    Facilitate the completion of mid-term evaluation report.

         15.    Complete required interviews and/or observations and meetings.

         16.    Obtain and complete an application for teacher certification.  Also, attend to other graduation matters.

         17.    Facilitate the completion of final evaluation report.

         18.    Make certain that Office of Teacher Education Services has received all evaluation reports by the date of the final seminar.

         19.    Ask University Supervisor and Cooperating Teacher for references.

         20.    Establish placement file in Career Center Office.

         21.    Find a simple, yet meaningful way to show your appreciation to your Cooperating Teacher for allowing you to work in his/her classroom.

SUMMARY CHECKSHEET FOR COOPERATING TEACHERS

        1.    Review upon receipt the intern information sheet and the internship handbook.

        2.    Work with intern to plan visitation/observation/participation schedule.  Allow as much observation and participation as possible before the internship actually begins.

        3.    Provide a desk or appropriate work space for the intern.

        4.    Provide books and materials for intern.

        5.    Introduce the intern as your “partner,” “team teacher,” and use Mr., Ms., Miss, or Mrs. when addressing her/him in front of the students.

        6.    Furnish intern with copies of the faculty and student handbooks.

        7.    Discuss and complete with the intern the Intern Expectations form. 

        8.    Provide a tour of the building.

        9.    Introduce intern to principal, assistant principals, secretaries, counselors, custodians.

      10.    Introduce intern to other members of the department, alerting the intern of each teacher’s special areas of expertise or talent and encouraging him/her to consult/observe them.

      11.    Explain attendance procedures and policies.

      12.    Explain fire, disaster, drill, and intruder procedures.

      13.    Explain procedures for being out of the room (e.g.,restroom, hall pass, library or media center pass, counselor pass).

      14.    Explain school and classroom grading policies and the format for keeping school and classroom records.

      15.    Explain school day schedules including lunch,
                assemblies, special classes.

      16.    Explain schoolwide and classroom discipline policies
                and referrals.

      17.    Work as a team in planning at first and then expect the intern to select objectives of the lessons, relevant teacher behaviors.

      18.    Observe intern teaching daily.  Leave the room for short periods of time early during his/her teaching and then for longer periods of time as he/she gains more experience and confidence.

      19.    Be constructive in criticism, providing and soliciting
                practical solutions to problems.

      20.    Avoid intervention when the intern is teaching unless:

               a.  a potentially dangerous disciplinary situation is
                     developing

               b.  the intern has lost self-control

               c.  the intern seeks your support

               d.  inaccurate information is conveyed and extensive
                    reteaching would be necessary

               e.  the intern has departed too far from the approved
                    lesson plan

      21.    Complete mid-term evaluation.  Conference with
                University Supervisor.

      22.    Complete final evaluation reports.

      23.   Conference with University Supervisor and intern
                regarding performance of intern.

      24.   Complete reference forms for Career Center, if asked.

SUMMARY CHECKSHEET FOR UNIVERSITY SUPERVISORS

         1.  Once interns are assigned, visit schools to meet Cooperating Teachers.

         2.  Review internship handbook.

         3.  Meet with interns to discuss policies and to respond to questions.

         4.  Collect Intern Information Sheet (Secondary/P-12 only) and Weekly Plan by the end of the second week of the semester.

         5.  Collect lesson plans weekly and complete at least two visits (announced) before mid-term.

         6.  Complete mid-term evaluation form and conference with intern and Cooperating Teacher.  Set performance goals as needed.

         7.  Periodically review internship notebook (Secondary/P-12) or journal (ECE/Elementary). 

 

         8.  Complete at least three more visits for a total of five (at least two unannounced).

 

         9.  Assist intern with growth toward performance goals.

        10. Complete final evaluation report.  Conference with
                Cooperating Teacher.

        11. Complete an evaluation form for each Cooperating
                Teacher with whom you worked.

        12. Collect all evaluation reports before the final seminar.

 

        13. Submit all observations and evaluation reports to the Coordinator of Clinical Experiences and Partnerships by the end of the semester.
