            The Alabama Department of Public Health has requested that all colleges and universities within the state require two doses of measles vaccine for students prior to enrollment in a program of study.  The University has adopted this public health requirement.  Please consult your physician regarding diagnosis of measles disease, evidence of measles immunity (Rubeola titer), or verification of two doses of measles vaccine.

            Attached are the Student Medical History Form and the Tuberculosis (TB) Risk Questionnaire.  Please consult your physician regarding completion of these forms.  The forms are required.  Submission of the completed forms are required before a prospective student is allowed to register for classes.

            Mail completed health forms to UM Health Center, Station 6275, Montevallo, AL 35115 or fax to (205) 665-8180.  If you have any questions regarding the Medical History Form or the TB Risk Questionnaire, please call the Health Center at (205) 665-6275.

 

International Students:  International students are required to have a TB skin test or chest x-ray done within the 12 months prior to attending the University.  Failure to complete the Medical History Form will delay processing the required immunization forms for admission to the University.
