Checklist of requirements for international students
 

Print this list to facilitate keeping track of forms and documents which are required by UM for admission and issuing of the I-20. When completing the application and other forms be sure to type or print clearly to help avoid delays in the admissions process.

_____1.	Application for admission, completed and signed with the required $30.00 fee.
_____2.	Completed medical form signed by a physician and TB Risk Questionnaire.
_____3.	Official TOEFL or IELTS score sent to UM.
_____4.	Affidavit of financial support and supporting documentation.
_____5.	Complete transcript of all secondary school study with appropriate English translations.
_____6.	Copy of passport or national identification card.
_____7.	Evaluation of Foreign Educational Credentials for college credit earned at a college or university outside the United States. (if applicable)
_____8.	Official transcript(s) from all US institutions attended. (if applicable)
_____9.	Transfer Clearance Form completed by a DSO of the school student is currently attending if within the US. (if applicable)
