Application for Graduate Study (PDF):  Students applying to  graduate study at the University of Montevallo should complete this application.   Please note this application has a $30.00 application fee that must be submitted along with the application. Applications received without an application fee will not be processed.

Alternative Fifth Year Evaluation Application (PDF):  The Alternative Master program offers students who do not have a Bachelor's Degree in education the ability to obtain both teacher certification at the Master's level along with their Master of Education degree. All students applying to the Alternative Fifth Year program will need to complete this application. Please note this application has a $25.00 application fee that must be submitted along with the application. Applications received without an application fee will not be processed.  In addition, students must also submit an application to Graduate Admissions & Records with a $30.00 fee.
