STUDENT HEALTH INSURANCE POLICY BEGINNING AUGUST
1, 2011
UNITED HEALTH CARE
(UnitedHealthcare)
The University of Montevallo will no longer provide health insurance for students. However, students will find information below regarding a health insurance policy provided by United Health Care that can be purchased if they choose to enroll in a health insurance program while attending the University of Montevallo as an undergraduate or graduate student.
International students who do not already have insurance that meets certain specifications are required to enroll in the United Health Care Insurance program.
Enrollment is easy and can be done online.
Students can pay their premiums online.
Who should consider enrolling?
Anyone who does not have health insurance.
Anyone who
has a chronic health condition and the
University’s AETNA policy was their
insurance provider last year.
(If you do not maintain insurance coverage,
your condition may be considered
pre-existing and coverage for the condition
may be denied.)
Anyone who
was under a physician’s care as of July 31,
2011.
(If you do not maintain insurance coverage,
the condition requiring care will be
considered pre-existing and coverage for the
condition may be denied.)
Visit the UnitedHealthcare website at http://www.uhcsr.com for additional information and to enroll.
NOTE: Students who opt to pay their premiums monthly are actually signing up for one month’s coverage. If a student misses paying a monthly premium, his or her coverage will lapse.
International students must go to the following website to complete a waiver form/card: https://studentcenter.uhcsr.com/montevallo
The waiver form must be completed by September 5, 2011.
