Address
Change
Students can easily
change their address through Student Resources
on the UM home page:
1. On the internet, go to UM home
page
2. Click on the forUM logo and log in
3. Click on the Banner tab
3. Select Banner Self Services
5. Select view Addresses and phone numbers. If
anything is incorrect select Update Addresses
and Phones at the bottom of the page. Enter your new
address.
Address changes may also
be done by filling out the
Address Change form in the Registrar's
Office or by faxing or sending a letter. The
letter needs to include the following:

student's name (as
it is currently on the computer)

UM ID (if Known)
-
the information that
needs to be changed (indicating if it is the
permanent or student address)

student's signature
(the student must sign the letter)

Name or Social Security
Number Change
Students must change their
name or social security number in the
Registrar's Office by completing the appropriate
section of the
Address Change/Name Change Form and
providing current identification showing
their new name.
Documentation of the change must be provided
before a name or social security change will
be made to the student's records. Contact
the Registrar's Office of more information.
