New International Students

The Alabama Department of Public Health has requested that all colleges and universities within the state require two doses of measles vaccine for students prior to enrollment in a program of study.  The University has adopted this public health requirement.  Please consult your physician regarding diagnosis of measles disease, evidence of measles immunity, or verification of two doses of measles vaccine.

The University also requires proof of a tuberculosis (TB) skin test or chest x-ray within the 12 months prior to enrollment. TB skin test results must be in millimeters.  International Students failure to complete the Medical History Form will delay processing the required Immigration forms for admission to the University. The attached Student's Medical History form has appropriate sections for this information.

Please consult your personal or family physician regarding completion of the Student Medical History Form.  The form is a requirement and compliance is mandatory for a prospective student to be eligible for registration and enrollment.
