Checklist of requirements for international students
 

Print this list to facilitate keeping track of forms and documents which are required by UM for admission and issuing of the I-20. When completing the application and other forms be sure to type or print clearly to help avoid delays in the admissions process.

_____1 Application for admission, completed and signed with the required $25.00 fee.
_____2 Completed medical report signed by a physician.
_____3 Official TOEFL score sent to UM.
_____4. Affidavit of financial support and supporting documentation.
_____5. Complete transcript of all secondary school study with appropriate English translations.
_____6. Evaluation of Foreign Educational Credentials for college credit earned at a college or university outside the United States. (if applicable)
_____7. Official transcript(s) from all US institutions attended.
_____8 Transfer Clearance Form completed by a DSO of the school student is currently attending if within the US.
