Intake Form Information

If you are planning on visiting the Learning Enrichment Center (LEC) for our free tutoring services, academic consultations, to attend any of the workshops offered, or to take part in any of the other services we offer, we ask that you fill out an Intake Form for our records and to help us better assist you. 

 

The LEC’s Intake Form can be opened and printed from the link below.  Please bring the completed form to the LEC, where we will meet with you to discuss your academic needs and find the help you need to become the successful student you want to be.
