If the woman who is receiving a bid is under 18, please see the Panhellenic Advisor for the correct form
 

2012 Recruitment Rules

Roster Removal Form
Use this form to remove members from your roster. Removals may be terminations, graduations, study abroad, etc.
 

Chapter Recruitment Schedule

Grade Release/Hazing Form
This form needs to be filled out each semester and submitted to the Panhellenic Office
 

Prospective Member Grade Release Form

Event Registration
This form must be filled out and filed with the Panhellenic Office at least one month prior to any Chapter event.
 

Fall 2011 Grade Ranking
 

Service Hours Form

This form should be turned in to the Student Life Office twice a year at the end of each semester.
